Our Services
Capacity Building & Achieving Goals
We work alongside you to build your capacity, increase access to your local community, and support you in achieving your personal goals. Through regular and purposeful check-ins, we adapt to your changing needs and ensure your supports continue to work effectively for you.
Quality, Monitoring & Support Coordination
We provide Level 1 and Level 2 Support Coordination for NDIS participants with self-managed or plan-managed funding. Our services include ongoing monitoring and reporting in line with the NDIS Code of Conduct and Practice Standards, ensuring you receive safe, high-quality support from your chosen providers.
Support Coordination That Works for You.
It begins with you—your lifestyle, your choices, and your control. We tailor supports and services to fit around what matters most to you, working alongside you every step of the way. Together, we ride the wave.
Plan Reviews & Ongoing Support
We assist you in preparing for your NDIS plan review and guide you through the review process, making sure your goals, progress, and support needs are clearly communicated and understood. This includes gathering and reviewing supporting documents and evidence for the best possible outcome.
Manage Your Supports & Budget
We help you manage your supports by connecting you with suitable providers and supporting you to monitor your plan expenditure if you are self managed or in collaboration with your plan manager. This ensures you can effectively manage your budget and get the most out of your NDIS plan for its full duration.
